The email

Both are optional. The grey text is what goes in if you leave them blank — type over it only when this release needs something different.

Release number

The packet number by default. Replace it with the yard's own release number when they issue one.

Where it is going

Nothing on file yet — normal if the pipe is still being coated. A line set to deliver will say “to be advised” rather than go out with a blank address.
